A pre-completed prepared application may be downloaded here.
You must fill in the blank lines on the application by clicking on each line or
checkbox and typing the data directly into the form. Do not modify any items that
are already completed for you as this may delay your application being approved.
When you are finished go to the upper left corner of the PDF reader and click the
little picture of a disk to save the application. A box will pop up and it will
ask you where on your computer you wish to save it. Saving it to your desktop will
be easiest.
When you are done filling out each part of the application and it is complete, print
off the final copy to your printer. Make sure that all 7 pages print correctly to
your printer and they are all legible. Laser printers are best, but inkjet will
work also.
Take the final copy to a notary and have them notarize it for you. If you wish,
you may make another copy for your records of the notarized version.
